Looking down at the top of the coin drop, you will see a small tab of metal on the right side of the coin drop, that extends downward and then bends to the left. The tip of this tab extends into the path of the coin. If you bend this tab up & out of the coins path, or remove it all together, you will see no further coin problems.
I think it was put there to guard against slugs and foreign coins. It seems if the customers put their coins in too fast, then the coins can jam up. I had a lot of similar coin problems, until I removed the metal tab. I have had no problems since then.

I have Dexter's too. I bent the tab out months ago, but still have occasional problems. Most of the time the customers get the coins to drop by pounding a bit on the machine, I've found that if they would hit the coin return they could get it back, but that's too obvious I suppose.
I think it's just poor design in the drop slide.
