Cash with paycheck stub and W2 and all taxes.

Use a payrole service I.E. paychecks, ADP get quotes from both and shop them, they usaly have work-comp plans, If your a CLA member there is a Paychecks diacount plan and CLA has a great Work-Comp Rate
